First you need to collect a list of documents including:
Any medical visits for the month printed from the Access Client Database (until we begin to enter these into Foothold)
The Activity Calendar (eventually this will be completed in Group Activities)
The Financial Status Report
Any unusual incident report summaries
Open the “Services” Menu.
Choose your program and then choose “Service Plans”.
Choose the person, and month you are running a report for and change the view from Daily to Monthly.
Choose “Daily Tracking Record”.
At the bottom of the page you will see the status key and list of objectives. Fill in the appropriate progress note code based on your plan for the next month.
To fill in the comments boxes to the right:
First, look at the information above and state how many times the objective was worked on this month.
Second, state the number of times the person worked on the objective at the highest level of independence shown.
Third, give a brief overall statement of how the person did that month.
Example:“Jane worked on this objective 16 times this month.She completed the objective 12 times with gestural prompts.Jane appears to enjoy working on this objective and is enthusiastic about completing it.”
**Hint- for those who have similar comments in the behavioral and emotional sections each month, it may be helpful to keep notes in a word document which can be pasted into this form.*Remember, if you use Chrome that the boxes can be enlarged for easier viewing.
Fill in any overall comments related to the person’s progress that month.
Choose “Continue”.
To print this report, use the print icon (DPD logo) on the top right of the page.
Next, go back to “Services” and choose “Progress Notes”.
Choose the person and the staff who is filling out the report.
Click “Continue”.
When you see the note index, click “continue” again.
You are now creating a monthly progress note. Change the selection from “General Chart Note” to “Monthly Note”.
In the “Include” Dropdown, choose “New DPD Consumer Monthly Report V.2”.
Choose “Continue”.
You will see the date range which should be reset to the beginning and end of the month you are reporting on.
Continue to fill out the report in the same fashion as the paper form.
Once finished, the system requires that a note be placed in the note box below the report. This can be simply a period. Click “SAVE NOTE”.
Once saved, return to the “notes index” and click on the date of the note to view it as a report and print it out.
22. *Please do not sign the note electronically at this time. Once it is signed it is locked and can no longer be edited.
Attach the Daily Checklist Monthly Summary/Progress Notes form, medical visits, calendar, financial information, etc. to the report as you have done previously.
